Signing up is easy. You may submit your application via our Shell Fuel Card Application Form (Digital).
Alternatively, you may download the Shell Fuel Card Application Form (English) (PDF, 3 MB) or Shell Fuel Card Application Form (Bahasa Malaysia & Chinese) (PDF, 3 MB) and prepare the required documents.
a) Company Application (Sdn.Bhd. or Berhad)
1. Completed Shell Fuel Card Application Form
2. Form 9 and latest Form 13
3. Latest 3 months bank statement
4. Latest Audited Report
5. Bank guarantee is required upon request only
b) Sole Proprietor or Partnership
1. Completed Shell Fuel Card Application Form
2. Photocopy of NRIC / MYKAD
3. Borang BE with bank payment slips / LHDN receipt of Business Registration Certificate, Maklumat Perniagaan & Pemilik & Form D/E
4. Latest 3 months bank statement / savings passbook
5. Bank guarantee is required upon request only
c) Government Account
1. Shell Fuel Card Application Form

How to Complete Your Fleet Details Information Form:
1. Select the Card Type you prefer : The Driver Card or The Non-Driver Card
The Driver Card
The Cardholder/ Driver's name, Company name, and the Vehicle Registration Number and Make will be embossed on the Card. The Card can only be used by the designated Driver for the designated vehicle.
The Non-Driver Card
Only the COMPANY name and the Vehicle Registration Number and Make will be embossed on the Card. It is most suitable if you have a pool of Drivers driving the same vehicle.
2. State the Drivers' name where applicable
3. State the Vehicle Registration Number
4. Indicate if truck tonnage is above 3.5 ton
5. Indicate if odometer reading is required
6. Specify the product and services allowed
7. Specify the purchase restriction for each card (number of transactions per day allowed, the daily, weekly and monthly purchase limits) from Purchase Restriction menu.
